i have a feeling that sometimes ardour does not "free" unused jack ports 
when loading a new session at runtime. it's not 100% reproducible, and i 
may be mistaken, but i think i sometimes run out of jack ports when i 
shouldn't be. those alleged "zombie" ports aren't visible in qjackctl, 
though...
and i've seen similar connection problems, sometimes with inserts.
maybe it's related to what you're seeing, maybe not. i'll keep an eye on it.
