this patch should do the following;

treat ranges as cd track markers when exporting to toc/cue files
treat markers as track indexes when exporting toc/cue files

add basic english cd-text to toc/cue files

add isrc, scms, preemphasis, performer, composer parameters to range
markers when they are cd tracks. 

the cd-text track name is the range name.
the cd-text cd title is the session name.

indexes that fall outside tracks will be ignored
tracks that start within another track will be ignored
non-cd-frame aligned marks will be rounded to the nearest cd frame in
the toc/cue file.

note that isrc uses capital letter only, so lower case isrc's will
confuse cdrdao/k3b.
