  * when naming ports, look for "holes" in the existing sequence of
      ports
  * fix crashing bug with comment window
  * major fix for audio rendering of regions on various layers +
      xfades. the rendering now happens in layer order rather
      than xfades always being last ("on top")
  * remove extra Gtk::Frame around buttons in IO selector

  * ardour/gtk: 0.558.1
    libardour:  0.858.0

notes: 

   the visual appearance of tracks where regions are placed
   "within" an xfade is wrong, and will be fixed very soon.

   if you remove a port, it should be the last port. my next
   commit will enforce this in the GUI.
