[ardour-dev] miscellaneous fixes patch.

this patch should do the following:

stop glitches in the audio output caused by process_output_buffers()
being interrupted mid cycle by using a write lock. this looks a bit
hackish, but it seems to work well.

cause the loading session to not overwrite it's own instant.xml.
(this fixes the click state and editor mixer state bugs. maybe more)

leaves the mixer window closed when a session is loaded or created (a
simple/stupid way to deal with mantis 0001043).
