   * changed automation pan lines to use correct model<->view mapping
        (untested)
   * make sure option editor's "follow playhead" button is in-sync
        with follow playhead setting
   * fix long-standing bug with MIDI request pool running out of
        memory.
   * convert from using pthreads condition vars and mutexes to FIFO's
        for audio-thread <-> butler-thread communication
   * fix export of 100% zeroes in 32 bit integer sample files
   * added MMC command when in master MMC mode for transport 
        startup (Deferred Play)
